Default BG check loading incomplete

Frank-
When I navigate to the Plusone site, first I get a classic certificate warning regarding the name of the holder. No biggie. The problem lies in the form itself not loading completely. I only get the headers, with no text boxes for my info. Should I contact Plusone directly? Is my current Carco BG check sufficient? So many questions!!

I myself, have filled out the background check but can't seem to find where I check the status. Any help would be greatly appreciated.
